lidia Pathways provide Transitional services which are items and expenses necessary and reasonable for a person to transition from an eligible setting to their own home or an integrated community supports setting.
Transitional services cover: 1. Essential furniture (e.g., bed frame, box spring, mattress, dining table and chairs, dressers, lamps, sofas) 2. Household supplies (e.g., cleaning supplies, dishes, drinking glasses, flatware, pots and pans, sheets, towels) 3. Lease or rental deposits 4. Moving expenses (e.g., moving vehicle rental, packing, unpacking) 5. Pest and allergen treatment of the home 6. Utility set-up fees and deposits 7. Window coverings.
A person is eligible to receive transitional services if they meet all the following criteria:
