We perform comprehensive assessments which include the client's medical history, an assessment of cognitive abilities, functional and emotional status, assessment of strengths, living environment, finances, and existing social support.
From our assessments, we develop a care plan for short-term and long-term goals. The Care Plan is tailored to include recommendations regarding medical and health care issues, community resources, housing options, financial and legal planning.
Our Care managers monitor the progress and milestones of the plan to ensure that it continues to meet the person’s goals. Care Plans are modified as necessary to meet those goals. Care managers are available 24/7 to assist in emergencies and discuss concerns. We monitor and evaluate the overall health and support services provided to our clients.
We provide coordination of services when multiple disciplines are involved. Coordination of services may include home health agencies, homemaker and companion services, adult day care, special transportation or assistive technology.
Our Care Managers educate family on self-directed supports and provide valuable information to make good decisions They also educate seniors and families on the benefits available to them including community, state, and federal programs.
Acting as a Health Care surrogate for clients without loved ones available or in-town, we advocate with health care and service providers to ensure that the client understands and approves the care or treatment provided.
We assist our clients in the relocation process between healthcare facilities or from a facility to the community. We make the necessary arrangements for facility admissions, transfers, and discharges. We research, evaluate, and arrange for senior living options.
As an Objective Third Party, we assist families in the thoughtful decision-making process, we facilitate family meetings regarding care, enabling families to reach consensus and care solutions, even with strained and difficult family dynamics.
