Relocation Service Coordination

Olidia Pathways is devoted to providing nursing home relocation services to their clients. These services provide coordination of activities to help a person who resides in an eligible institution gain access to medical, social, educational, financial, housing and other services and supports that are necessary to move to the community.

The staff at Olidia Pathways work with individualized relocation plans designed to meet the needs of each client and promote successful nursing home relocation. We create a customized and unique transition plan tailored to each client.

Relocation service coordination targeted case management (RSC-TCM) provides coordination of activities to help a person who resides in an eligible institution gain access to medical, social, educational, financial, housing and other services and supports that are necessary to move to the community.

Those institutions are: Hospitals
Nursing facilities (NF), including certified boarding care facility
Intermediate care facilities for persons with developmental disabilities (ICF/DD)
Regional treatment centers (RTC) that provide inpatient services to people who currently receive Medical Assistance (MA).

These individuals are moved to: Group Homes
Independent living Apartments
Assisted Living communities, and
Personal homes

What We Provide

ELIGIBILITY FOR SERVICES

Here are the requirement for relocation services:

ELIGIBILITY CRITERIA

To receive RSC, the person must meet all of the following criteria:
1. Be eligible for and receiving MA
2. Reside in an eligible institution at the time the person receives RSC
3. Be closed to any home and community-based services (HCBS) waivers due to a stay in an eligible institution
4. Choose to live in the community
5. Choose to receive RSC.

ELIGIBILITY ASSESSMENT

The person needs an assessment to determine and document eligibility:
1. If the person has had a MnCHOICES or long-term care consultation (LTCC) assessment, the lead agency can authorize RSC within 365 days from the date of the assessment
2. If the person has had a developmental disabilities (DD) screening, the lead agency can authorize RSC within six months from the date of the assessment, as long as RSC is listed as a current service.
