TRANSITIONAL SERVICES

lidia Pathways provide Transitional services which are items and expenses necessary and reasonable for a person to transition from an eligible setting to their own home or an integrated community supports setting.

Covered services

Transitional services cover: 1. Essential furniture (e.g., bed frame, box spring, mattress, dining table and chairs, dressers, lamps, sofas)
2. Household supplies (e.g., cleaning supplies, dishes, drinking glasses, flatware, pots and pans, sheets, towels)
3. Lease or rental deposits
4. Moving expenses (e.g., moving vehicle rental, packing, unpacking)
5. Pest and allergen treatment of the home
6. Utility set-up fees and deposits
7. Window coverings.

Eligibility for Transitional Services

A person is eligible to receive transitional services if they meet all the following criteria:

AGE AND WAIVER ENROLLMENT
Age 18 or older and enrolled on the Brain Injury (BI), Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC) or Developmental Disabilities (DD) waiver, or reasonably expected to enroll within 180 days
TRANSITION TO COMMUNITY
Moving from an eligible setting to their own home or an integrated community supports setting
INABILITY TO GET FUNDING
Not able to access transitional services from other funding sources (e.g., community nonprofit organizations).
NOT FINANCED SETTING
Moving to a setting where these items and expenses are not normally furnished

AFC
Adult Foster Care Home (AFC)
Adult Foster Care. Adult Foster Care (AFC) is a type of housing for people with disabilities and seniors who need some daily care, but do not need skilled nursing care. Adult Foster Care is sometimes called Community Residential Settings (CRS).
Hospitals
Hospitals
Hospital, an institution that is built, staffed, and equipped for the diagnosis of disease; for the treatment, both medical and surgical, of the sick and the injured; and for their housing during this process.
IMD
Institution for Mental Diseases (IMD)
Focusing on portability and usability, this feeding pump can provide intermittent or continuous dosing while providing up to 15 hours of mobile nutrition via a rechargeable battery.
ICF/DD
Intermediate Care Facility for persons with Developmental Disabilities (ICF/DD)
An intermediate care facility for persons with developmental disabilities (ICF/DD) is a licensed supervised living facility. These facilities provide services to people who have developmental disabilities or related conditions.
SN
Skilled Nursing facilities (SN)
A skilled nursing facility is an in-patient rehabilitation and medical treatment center staffed with trained medical professionals. Skilled nursing facilities give patients round-the-clock assistance with healthcare and activities of daily living (ADLs).
HWS
Registered Housing-with-Services Establishment (HWS)
A HWS is an establishment providing sleeping accommodations to one or more adult residents, at least 80 percent of which are 55 years of age or older, and offering or providing, for a fee, one or more regularly scheduled health-related services or two or more.
Unlicensed Setting
Unlicensed Community Setting
Unlicensed or Community setting are private homes, family homes or integrated community supports settings.
CL
Customized living (CL)
Customized living (CL) is an individualized package of regularly scheduled, health-related and supportive services provided to a person age 18 years or older who resides in a qualified setting.
