CARE PLANNING

From our assessments, we develop a care plan that is tailored to include recommendations regarding medical and health care issues, community resources, housing options, financial and legal planning.

COORDINATION OF CARE

We use a multidisciplinary team of clinical managers, social workers, therapists, physicians, and other stakeholders who share information and collaborate to deliver a holistic and well executed plan of care

ADVOCACY

Acting as a Health Care surrogate, we facilitate the communications between physicians and families making sure that appropriate family members are updated on care services

RELOCATION

We assist our clients in the relocation process between healthcare facilities or from a facility to the community. This includes facility admissions, transfers, and discharges.

HOME CARE SUPPORTs

We oversee and provide support and coaching for home and family caregivers to ensure consistency and quality of care. We also coordinate medical appointments and consultations.

INSURANCE SUPPORT

We help and assist our clients as they navigate the complex healthcare and insurance maze. We negotiate with insurance companies for needed eligible services. We review Claims and Medical Bills.

OUR CLIENTS

Our range of senior care management services are especially beneficial to:
1. Seniors who want to remain independent and live in the comfort of their own homes
2. Seniors with relatives living out-of-state or travel for extended periods
3. Families who need assistance with coordinating the care of their loved ones Elderly

Here are some of the features of our clients:
1. Many are frail and in need of assistance or supplemental support
2. Those who have a chronic illness, such as Parkinson's, cancer, have just suffered a stroke, etc.
3. Clients with Alzheimer's disease or other forms of dementia
4. Those who are recently bereaved and in need of grief counseling
5. Clients who are in transition, perhaps moving from one living arrangement to another
6. Clients who have no living or capable family member to act as an advocate for them

We serve clients with care management needs from a variety of conditions:
1. Disabled adults.
2. High-functioning developmentally disabled.
3. Adults with psychiatric disorders such as schizophrenia or bi-polar disorder.
4. Those with substance abuse issues, such as to prescription medications or alcohol dependence.
5. Adults with debilitating chronic illness such as Multiple Sclerosis or Parkinson’s Disease.
